This Consumer Health Data Privacy Notice supplements The Haven Texas's general Privacy Policy and applies
only to the extent a state consumer health data privacy law applies to information that is not already regulated or
exempt as protected health information under HIPAA, HITECH, 42 C.F.R. Part 2, or another applicable healthcare
privacy law.
Consumer Health Data We May Collect
Depending upon your interaction with The Haven Texas, consumer health data may include information
concerning:
 substance use or substance use disorder;
 alcohol use;
 an individual's request for or interest in treatment;
 physical or mental health;
 symptoms or medical conditions;
 medications;
 treatment history;
 healthcare services sought, received, or considered;
 health insurance or coverage;
 information concerning admission to a healthcare facility;
 information concerning transportation for healthcare;
 information concerning aftercare or continued treatment;
 information identifying an individual as seeking healthcare or substance-use-related assistance; and
 information from which health status or treatment-seeking activity may reasonably be inferred.
